The key to managing complex odontogenic infections with loss of alveolar bone and sinus perforation is careful debridement and a 3-layer socket graft technique to restore defect and manage the perforation- all at the same time. Generally, in 6 months the infection and related sinusitis resolves and the site is ready for implant replacement.
And let’s not forget about the critical importance of 3-D diagnostics (CBCT). Using a conventional 2-D x-ray will not clearly show the extension of the defect and leads to an unanticipated ‘surprise’ during surgery.
