How can I save time by pre-registration?
Save time by completing your forms prior to your visit. You may register online (link) or print (link) them and sent via fax (301) 654-7050 or email ([email protected]). There are three necessary forms:
Patient and guarantor information
Medical history form
HIPAA form

What X-rays are necessary?
X-rays are necessary for proper diagnosis and planning for your surgery. There are three types of X-rays:

  1. Panorex: indicated for wisdom teeth, multiple extractions, dental implants, bone grafting, and some biopsy procedures
  2. Peri-apical X-ray (small dental X-ray): Adequate for non-wisdom teeth extractions.
  3. CT-Scan: Indicated for some bone grafting procedures, some dental implant procedures, jaw cysts, and other forms of pathology.


Can I use previous X-rays taken by my dentist?
We can use X-rays taken within 6 months of your planned oral surgery
Your dentist can send your X-rays to us electronically or your may bring a copy with you to your appointment
X-rays must of good quality and diagnostic
We can take any necessary X-rays in our office using state-of-the-art digital technology

Do I need a referral form?
Yes. Referral form is a medico-legal document reflecting your dentist’s recommendations and exactly what procedures are being requested. It provided clear communication between your dentist and our office. The referral form may be send in three ways:

Bring a referral form with you on your appointment
If you do not have a dentist, you do not need a referral form.

Can I send an X-ray for online evaluation?
Yes. We will be happy to evaluate your X-ray and provide you with some information about your desired procedure. However, clinical examination is an important part of the over all diagnosis and recommendations.

Do you offer dental CT scans?
Yes. We have Kodak CS 9300 dental CT scan right in our facility. The process takes about one minute and images are available immediately for diagnostics. Learn more about dental CT scans.

How much is a dental CT scan?
Dental CT scan cost is $275.00. This fee is all CT sizes and fields of view. The images can be placed on a disc and shared with your dentist or other healthcare providers.

When is dental CT scans recommended?
Dental CT scan is recommended for dental implant planning, third molar surgery, and various pathologic conditions. It provides 3-dimensional view of teeth, jaw bone and supporting structures to help the oral surgeon and your dentist with diagnostic and planning of your treatment.

Information on dental CT radiation.
As with any x-ray imaging, there is some exposure to radiation. However, our CT scan emits significantly less radiation that medical CT scans and only slightly higher than a dental full mouth series of X-rays or a panoramic X-ray. How to email an X-ray?
X-rays can be emailed as a jpeg image. If you have an existing X-ray, contact your dentist and have them send you the image.If you have it in a film format, take a photo of it with a digital camera or phone and then email the image.
