What is referral form?
A referral form is a communication tool often provided by your dentist or physician requesting the specific procedures to be performed.


Why do I need a referral form?
Communication is critical between your dentist and our office to assure the right procedure is performed as recommended. This is a legal document and important part of the communication between our office and your doctor. It will become part of your records. A referral form is not necessary If you don’t have a dentist and are requesting a procedure yourself.


Where do I get a referral form?
Your dentist or physician should provide you with the referral form containing all relevant information about the recommended procedures.


How are referral forms sent?
Referral forms may be sent by:

  • Online- click here for form
  • Fax: Our referral form is available and most dentists have them. If they don’t, we’ll be happy to send them.
  • Bring on your appointment: If you were given a paper referral form, you may bring it with you to your scheduled appointment.
